Public Information Request

A request for public information does not have to be written on the form below, however, the form does provide information regarding public information law. We have also provided a link to the attorney general's website for your convenience.

The Town of Bayside provides the following ways to submit your public information request.

Hand Delivery or Mail

You may request a form from our government offices and hand deliver or mail it.

Written Form

You may submit a request to any government employee in any written form, including by electronic mail, fax, or letter.

Good to Know

Please note that for a public information request to be valid, it must be submitted in writing. In writing means email, letter or handwritten.
